Unfortunately, I am unable to accept insurance as payment for services. Upon payment, which is due at the time of service, I will provide you with an appropriate receipt so that you can submit a claim for reimbursement.

DIRECTIONS TO PENNSYLVANIA OFFICE
From Route 80 East:
Take Route 80 East to exit 284 / Route 115. Go North on Route 115 towards Blakeslee, about 1 1/2 miles. Follow instructions below from Blakeslee Corners.
From Route 940 at Blakeslee Corners:
Go Eastbound on Route 940 for about 5 miles. You will cross Pocono Lake (water on both sides of the road) and go up a gentle hill. On the left you will see mounds of construction material at Pocono Lake Supply. On your right you will see a + sign and a sign indicating a fire station. Make a quick left onto Old Route 940.
Note!! - If you pass the Emergency Services building sign on the right, you have gone too far.
Go about 1/2 mile on Old Route 940 and slow down as it begins to curve to the right and you pass another road, Third Street (poorly marked) which intersects from the left. As you curve to the right you will see a sign that says "Watch Children". The office is in a green house that is on the left immediately after the intersection. Please do not use the first driveway. Instead, continue slowly on Old 940 until you see the next driveway on your left, marked with a very large boulder and make a left into that driveway. Make your first left and drive a short distance to the patient parking area. Come up the path and ring the bell to enter the waiting room. Please ring the bell outside the actual office so that I know you're here.
Note!! - If you pass the American Legion Hall on the right with the Army tank on the lawn, you have gone too far.
From Route 84 West:
Follow Route 84 West to 380 South / Pocono. Exit at Route 940 and turn right onto Route 940 going West. Follow directions from Route 940 West below.
From Route 80 West:
Take Route 80 West to 380 North / Scranton. Take the first exit Mount Pocono / Route 940 and make a left at the top of the ramp onto Route 940 going West. Follow directions from Route 940 West below.
From Route 940 West at Intersection of Route 380:
Go approximately 4 miles until the second traffic light which is marked Route 423. Go through the intersection and almost immediately bear right onto Old Route 940. Note: The ice cream store on new Route 940 should be on your left side if you are on Old Route 940. Look at your odometer and begin to measure 1 mile.
You will pass a school on your left opposite a cemetery on your right. Further on, you will see Stony Hollow Road merging from the left.
Almost immediately on your right you will pass Pocono Lake Animal Hospital and on your left you will pass an Army tank / American Legion building. Approximately 100 yards up on the right-hand-side make a right turn into the driveway marked with a very large boulder and then make your first left and drive a short distance to the patient parking area. Come up the path and enter the waiting room. Please ring the bell outside the actual office so that I know you're here.
Note!! - If you see another driveway, marked with a small boulder, or a road curving to the right, Third Street, you have gone too far.
DIRECTIONS TO NEW YORK OFFICE
By train from Grand Central Station:
New Rochelle is easily reached via Metro North trains from New York City. Cabs are usually standing by. If you need to call for a cab, V&O City Taxi can be reached at: 888-633-4501 OR 914-633-4500.
From Hutchinson River Parkway Northbound:
Take Exit #5 City Island and proceed to traffic circle. Go ¾ around the circle and exit at Shore Road North. Continue driving on Shore Road until the third* light and make a right onto Drake Avenue (*not including the flashing yellow light). Follow instructions “From Drake Avenue” below.
From Interstate 95 Northbound:
Take Exit #15 and make a right at the bottom of the exit ramp. Go under the underpass and in approximately 300 feet, while passing “Home Depot” on your right, make the first right turn onto Weyman Avenue. Once you’ve made the right you should have “CVS” and “Aaron’s” on your left and “Home Depot” ahead on your right. On Weyman Avenue you will go through several lights before the avenue takes a downward slope and intersects, at a light, with Pelham Road. Make a left on Pelham Road and proceed to the first traffic light at Drake Avenue. Make a right onto Drake Avenue. Follow instructions “From Drake Avenue” below.
From Drake Avenue:
Continue to the end of Drake Avenue until you see the signs for Wright Island Marina and Sea Breeze One Restaurant. Continue straight past those signs and past “Customer Parking” on your right, following the driveway as it curves to the left. Pass through the upswung gate that says “Boat Owners Only”. Park as close as you can to the left side as you face the water. When you arrive, please call the office at 914-636-0099 so I can escort you to the office.
